Exciting new roles for Social Prescribing Link Workers

Hours: various full time (37 hrs pw) and part time roles available including Team Leader role
Location: Flexible across Fylde Coast with homeworking and travel
Salary: £22,585 - £28,300pa (pro-rata for PT roles)
Post status: Fixed term until 30th June 2021 (continuation subject to funding)
LETSconnect is a collaboration between the Voluntary, Community, Faith and Social Enterprise sector (VCFSE) and Primary Care Networks (PCNs) to deliver Social Prescribing across Blackpool, Wyre & Fylde. Citizens Advice Blackpool, the Volunteer Centre Blackpool, Wyre and Fylde and the CVS are working in partnership with Primary Care Networks covering Wyre and Fylde, Blackpool South, North and Central to deliver social prescribing to their patients.
LETSconnect have exciting opportunities for Social Prescribing Link Workers to join the team. LETSconnect is a service to help people improve their general health and wellbeing their way.  Through non-medical sources of community support, LETSconnect offers the chance to find out about the range of activities that are available locally to help move towards healthier, active, more connected lives.  These include; community groups, advice and information service, self-help groups, sporting activities, leisure pursuits, lunch clubs, special interest groups and much more.
The main duties of a LETSconnect Link Worker include developing relationships in the health and social care sector and supporting individuals to access voluntary and community opportunities.

This post will involve contact with vulnerable adults and is therefore exempt from the Rehabilitation of Offenders Act 1974 and all convictions must be declared, a DBS check will be undertaken.
